Before Work Begins

Consult with the site manager or delegate on the work to be done including the location and expected duration of work.
Utilise the WPCG Activity Matrix to identify all authorisations required.
Conduct a thorough inspection and assessment of the job site and surrounding conditions.
Commence the Work Clearance form, accurately detailing the work to be done and tools or equipment to be used.
Commence the Work Permit form, accurately detailing the work to be done and tools or equipment to be used.
Complete relevant risk assessments including JSAs and/or SWMS.
Specify hazards and required controls and safety measures.
Specify monitoring requirements including frequency of inspections.
Complete Minimum Controls Checklist(s) as required.
Complete High-Risk Certificate(s) as required.
Consult the Permit Receiver on all aspects of the Work Permit and provide them with an opportunity to contribute to the permit content.
Ensure all relevant authorisation documents have been completed and cross-referenced, including:
  • The Work Permit,
  • Relevant High-Risk Certificates,
  • Any supporting documents referenced on the Work Permit (e.g. lift-plan, isolations record, etc.), and
  • A WPCG Work Clearance.
Finalise documentation and obtain the site manager or delegate's signature.
Issue the Work Permit and obtain the Permit Receiver's signed acceptance of all conditions.
Sign the Work Permit to confirm understanding and acceptance of all conditions and requirements.

Conduct a toolbox talk with all personnel and sub-contractors working under the Work Permit or Work Clearance.
Ensure all personnel understand all hazards, controls and safety requirements.
Ensure all personnel understand the tasks to be carried out.
Ensure all plant and equipment to be used, including contractor equipment, is checked before use.
Ensure all personnel understand the risk assessment, Work Clearance, Work Permit and any other applicable authorisations, and have signed their acceptance on these documents.
Ensure work area is set up correctly and all relevant hazard controls are in place.

During Work

Remain physically present at the work site while work is being done.
Remain aware of potential hazards, monitoring and maintaining all required controls.
Ensure all personnel adhere to the conditions of the Work Permit including the presence of safety standby and fire watch roles if applicable.
Supervise work conditions to ensure they remain safe and within scope and conditions of Work Clearance, Work Permit, risk assessment and any other active authorisations.
Immediately stop work and secure the site if any of the following occur:
  • New hazards arise
  • Site conditions change
  • Work conditions change
  • The scope of work changes
  • Controls change or are not effective
Ensure you are contactable and available should any conditions change.

Immediately stop work and secure the site if any of the following occur:
  • New hazards arise
  • Site conditions change
  • Work conditions change
  • The scope of work changes
  • Controls change or are not effective
Contact the Permit Officer and inform them of the changed conditions.
Assess new conditions and identify additional procedures, authorisations and/or controls required.
Amend Permit and provide a detailed brief to Permit Receiver and Endorser on revised conditions of the Work Permit.
Review, verify and endorse the amended Work Permit at the work site.
Inform personnel of changes to the Work Permit, including additional hazards, controls, monitoring and authorisation requirements. Confirm their understanding before resuming work.
Confirm the requirements of the Work Permit have been communicated to all workers.

Ensure the site is clean, safe and secure.
Complete End of Day Sign Off section on Work Clearance.
Ensure the site manager or delegate sign the Work Clearance End of Day Sign Off.
Complete daily close out on Work Permit.
Communicate the status of any remaining hazards, equipment or isolations.

Commence the Work Clearance form, accurately detailing the work to be done and tools or equipment to be used.
Conduct a thorough inspection and assessment of the job site and surrounding conditions.
Complete relevant risk assessments including JSAs and/or SWMS.
Finalise the Work Clearance and obtain the site manager or delegate's signed authorisation.
Confirm that the Work Permit is still valid before endorsing. If a Work Permit expires before the work has been completed, a new Work Permit must be issued by the Permit Officer.
Verify the scope of work and site conditions remain appropriate for the Work Permit conditions and controls.
Confirm the Permit Receiver understands the intent of content, conditions and controls listed in the Work Permit.
Ensure any gas testing required by the Work Permit or associated Certificates is completed by an Authorised Gas Tester.
Ensure the area and relevant equipment is safe for work to continue.
If conditions are not met or have changed beyond the scope of the Work Permit, do not endorse the Work Permit or resume work. Notify the Permit Officer if this occurs.
If all conditions are consistent with the Work Permit, endorse the Work Permit by signing and completing relevant fields of the Daily Close Out and Endorsement of Permit section.
Ensure all personnel understand all hazards, controls and safety reqirements.
Confirm the requirements of the Work Permit have been communicated to all workers.

Completion of Work

Confirm that all equipment has been removed, the site cleaned and made safe.
Complete End of Day Sign Off section on Work Clearance.
Ensure the site manager or delegate sign the Work Clearance End of Day Sign Off.
Complete the Work Permit Completion and Close Out section of the permit.
Ensure the site manager or delegate sign the Work Permit to confirm the activity is complete.
