What health forms do I need to send? By when?

New York State requires that all students provide us with information about their health and immunization history. This requirement protects your health and that of other students and helps us better serve your health needs while you are at Ithaca College. All healthcare records are confidential and are retained separately from other college records.

Forms are available on the Student Health Services website and include:

  • Health History
  • Immunization Record
  • Measles/Mumps/Rubella
  • Meningitis Response
  • Notice of Privacy Practices
  • TB Screen-Risk
  • Emergency Contact
  • Minor Consent to Treat*
  • Minor Meningococcal*
  • Physical exams are encouraged but not required

*Required only if you will be younger than 18 as of June 1, 2023

Each completed form must be uploaded or mailed by June 1 and as indicated on the form. (You may also choose to upload documents to your MyICHealthCenter patient portal, which you will use your official Ithaca College username and password to access.) Please note that it may take health center staff up to 3 weeks to process your forms; if you receive an email saying a form is incomplete but you already submitted that form, you can disregard the email. 

Please be aware that students will not receive their move-in timeslot until their all health certification forms have been received and verified.

You can email ICHealthCertifications@cayugamed.org or call 607-274-3177 to confirm that all of your health form requirements are complete or if you have any questions about how to complete them. Health center staff are happy to assist you through this process.
