To help us provide you with the best possible support for your event, including adequate scheduling of the Events Staff and facilities, this form must be completed at least 4 WEEKS PRIOR TO YOUR EVENT.

Please Be As Complete As Possible
Your event will be staffed according to the information you provide, and your Event Services staff may not be able to accommodate last minute requests or changes. This includes not having audio staff for recording or talk mics, or crew to change setups during performance.
Streaming
Your event will be live-streamed, recorded for on-demand presentation, both, or neither based on the option you selected when you submitted your event date. Your choice is listed on the master calendar for your event. 
Music Rentals *
Our ASCAP/BMI/SEASAC blanket licenses cover us for everything except theatrical works and music rental works. Therefore, royalties for rental music must include the rights to broadcast (live-stream) and make available after the live-stream is over (on-demand).
