MHS Remote Counseling Check-In Request
Students, the MHS Counseling Department is thinking about you during this time.  We want to be available to you and connect with you when needed, even through remote means.  Please complete this form if you would like to schedule a time to check-in with your School Counselor.  This request link will be available Monday-Friday, 8 am-2 pm.  Upon submitting a request, your Counselor will work to follow-up with you in the next few days, but will not be able to respond immediately.  In order to meet the needs of all students, check-ins may be kept to a minimal amount of time.

This form is NOT to be filled out in the event of an emergency.  If you are in need of immediate assistance, or are in a crisis situation, please do one of the following:
