Centennial HS Graduated Student Transcript Request Form **Classes of 2019-2023 ONLY**

****Howard County Public Schools will be closed for Spring Break from March 25, 2024 through April 1, 2024. We will return to processing requests after we return to the office on April 2, 2024.****

This form is not intended for current HCPSS students.  Please contact your school counselor if you are a current HCPSS student for the proper transcript request procedure.  

For Class of 2018 and earlier, please email your request to student_transcripts@hcpss.org.

There is a $5.00 fee for each requested graduate transcript.  Requests will not be processed until payment is received.  Payment is available through the following link: https://osp.osmsinc.com/HowardMD/BVModules/CategoryTemplates/Detailed%20List%20with%20Properties/Category.aspx?categoryid=DA041.

Release of Student Records
The law requires that schools receive written permission signed by an adult age 18 or older to release records.  By completing this form, you are verifying that this form has been completed with your parent/guardian(s) approval if you are under 18 years old.  Please note that when you complete and submit the Transcript Request google form you are giving HCPSS permission to send your transcript through email to a third party.
