Basic Needs Assistance Request Form


Please fill out this form to connect with Student Care and Advocacy if you are a Penn State student, or if you know of a Penn State student who is facing challenges with access to adequate food, stable housing, or other basic needs.

 

Please Note:

  • This form is not intended for reporting urgent matters. If you are experiencing a crisis or there is an immediate threat of harm, violence, or substantial disruption, please call campus police or 911 or utilize our mental health crisis resources, including by not limited to the 24/7 Penn State Crisis Line at 1-877-229-6400.
  • This form is not reviewed after office hours, on weekends, or during University holidays or breaks.
  • If you are concerned about a student for reasons not connected to their basic needs, please submit a general CARE Report Form
